Botox® for chronic migraine

Botox® is a licensed preventive treatment for chronic migraine in the UK and Ireland. In appropriately selected patients, it may reduce the frequency, severity and duration of migraine attacks, leading to improvements in quality of life and day-to-day functioning.

Trigger point Injections

Trigger point injections using local anaesthetic, with or without a corticosteroid, may provide short- to medium-term pain relief for selected patients with myofascial pain syndrome. In carefully selected cases, Botox® may also be considered, particularly when symptoms are persistent or have not responded to more conservative treatments. These treatments are most effective when combined with a comprehensive rehabilitation programme, including stretching, exercise, posture correction and physiotherapy.

Stellate ganglion block

This specialised injection is used to relieve pain and other symptoms affecting the face, neck, upper chest or upper limb that may be associated with overactivity of the sympathetic nervous system. It may be considered for selected conditions, including complex regional pain syndrome (CRPS), post-traumatic pain, certain neuropathic pain conditions, and persistent symptoms related to sympathetic overactivity.

Nerve blocks for facial pain

Nerve blocks are effective treatment option for selected facial pain conditions, including trigeminal neuropathy, temporomandibular disorders (TMD), persistent idiopathic facial pain (formerly known as atypical facial pain), and bruxism. Nerve blocks may reduce the frequency, severity and duration of facial pain episodes.

Botox® for oro-facial pain

Botox® is an emerging treatment option for selected facial pain conditions, including trigeminal neuropathy, temporomandibular disorders (TMD), persistent idiopathic facial pain (formerly known as atypical facial pain), and bruxism. Botox® may reduce the frequency, severity and duration of facial pain episodes.
 
 
 

Sacro-iliac joint injection

This is a valuable procedure used to diagnose and treat pain arising from the sacroiliac joints and the posterior pelvic ligaments. The sacroiliac joints are recognised as a potential source of pain in up to one-third of people with chronic low back pain, particularly when symptoms arise from the lower back, buttock or groin. The injection may help confirm whether the sacroiliac joint is contributing to your pain and, in some patients, provide temporary pain relief to support rehabilitation and improve function.

Lidocaine infusions

Intravenous lidocaine infusions may provide relief in patients with refractory chronic pain when other evidence-based treatments have not provided adequate relief or have not been well tolerated. Responses and longevity of benefit vary considerably between individuals.

Epidural steroid injection

Epidurals are recommended for conditions such as acute sciatica, lumbar or cervical radicular pain (nerve root pain), and spinal stenosis in appropriately selected patients. The aim is to reduce inflammation around the affected nerve, relieve pain and improve function.

Radiofrequency denervation of cervical facet joints

This minimally invasive procedure is used to treat well-localised, one sided chronic neck pain arising from the cervical facet joints. In general, this procedure is more likely to be effective in patients with well-localised pain affecting a single area, rather than widespread or multisite pain.

Radiofrequency denervation of lumbar facet joints

This minimally invasive procedure is used to treat well-localised chronic low back pain arising from the lumbar facet joints. In general, this procedure is more likely to be effective in patients with well-localised pain affecting a single area, rather than widespread or multisite pain.

Dr Niranjan Chogle

Dr N Chogle is a fully trained Consultant in Pain Medicine & Anaesthesia. He is based in the Ulster Hospital as a Consultant in Anaesthesia & Pain Medicine.

Dr Niranjan Chogle's private practice is based at three clinics:

  • Kingsbridge Healthcare Group, Belfast
  • Ulster Independent Clinic, Belfast
  • Orthoderm Clinic, Hillsborough

He sees both insured and self-paying patients seeking assessment and treatment for chronic pain.

Dr Chogle graduated in India, where he completed his training in Anaesthesia and Cancer Pain Management. He moved to the UK in 2002 to undertake further specialist training and was appointed as a Consultant Anaesthetist at Ulster Hospital in 2008. He subsequently completed an additional year of Advanced Pain Medicine training in Aberdeen, Scotland.

His specialist interests include facial pain, temporomandibular disorders (TMD), migraine, whiplash-associated disorders, cancer pain, and complex pain conditions such as fibromyalgia and myofascial pain syndrome.

In appropriately selected patients, he offers a wide range of minimally invasive procedures to help manage facial, neck and back pain, potentially reducing the need for ineffective medications and avoiding irreversible surgical interventions.

He works closely with orthopaedic, general, and maxillofacial surgeons to help prevent and manage persistent pain following surgical procedures.

Dr Monica Chogle

Dr Monica Chogle is a fully trained Pain Consultant practicing in Northern Ireland.

Dr Monica Chogle's private practice is based at:

  • Kingsbridge Healthcare Group, Belfast
  • Medical Associates, Belfast

Dr Chogle graduated in India with an MBBS degree, followed by postgraduate training leading to an MD in Anaesthesiology. She subsequently completed further higher specialist training in Cancer Pain Management.

She undertook further specialist training in the UK and was awarded a Certificate of Completion of Training (CCT) in Anaesthetics. She also completed Advanced Pain Medicine training in Northern Ireland.

She has been practising Pain Medicine at Antrim Area Hospital since 2011.

She specialises in the management of low back and neck pain and has a particular interest in widespread pain conditions such as fibromyalgia, which can be challenging to treat. Her specialist interests also include female pelvic pain and chronic post-surgical pain. She is trained in the use of Botox® injections for chronic migraine and myofascial pain.

She adopts a biopsychosocial approach to the assessment and management of chronic pain. She played a key role in developing the Pain Management Programme (PMP) within the Northern Health and Social Care Trust.
